Each of our surgeons sees patients on Tuesday and Thursday mornings, but we will make effort to accommodate your schedule as possible. The institute participates in a large number of insurance plans; however you will need to verify our participation with your particular plan before you come in for an appointment. If you are on a managed care plan, please confirm that we have received prior authorization for your visit. As a new patient, you will be asked to bring all X-rays/MRIs/studies that have been performed relating to your visit with us. If time permits, a patient information packet will be sent to you. It contains instructions, as well as a list of items to bring with you to your appointment. We make every effort to see our patients promptly. Unfortunately, emergencies do arise, and often, we fall behind schedule. Please allow yourself two hours for the visit. Your patience is greatly appreciated.

What the patient should bring to the first visit...
  • Completed Patient information packet
  • Insurance Card(s)
  • Workman's Compensation Information if applicable
  • Driver's License
  • All X-rays, MRIs, Myelograms, CTs
  • Any previous medical records, especially test results (i.e., EMG/NCV, Pain Management documentation)
